Thank you for considering Further Chiropractor. Here is what you'll expect when you visit our office for the first time:

Paperwork
When you first visit our offices, you'll be asked to fill out paperwork that will help guide us in your treatment. The forms ask for general information about yourself and your medical history and condition. If you were in an accident, we ask for additional information including employer info, information about the incident and any medical treatment & insurance information related to the accident. Please be prepared to provide as much information as possible to help with your treatment. You may fill out the form upon your first visit or complete them ahead of time by downloading a PDF copy here and faxing or emailing it. Click here to download the form.

Documents
Bring your health insurance card, any current medications and any recent x-ray/MRI reports pertaining to your visit or make sure that your referring physician has sent all of your pertinent information to us, including a referral if necessary. Any co-payment required by your health insurance will be expected at each visit.

Consultation
Once the required paperwork has been completed, you will have a consultation with the doctor to discuss any health problems, concerns, as well as treatment options. The doctor will ask a series of questions related to your current condition. Clear and honest communication is vitally important to your treatment. Provide as much information about your health conditions on the paperwork or write them down and share them with the doctor.

X-Rays & Examination
After the initial consultation, a number of specialized tests will be performed to determine which tissues/joints are affected and to what extent. Your specific condition may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you. 

Treatment
After we have assimilated enough information, we will determine a treatment plan that will best serve your needs. Treatments may include chiropractic adjustments and/or physical therapies. The treatment plan will include instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.

Follow up Appointment
After your initial visit, you will be asked to schedule a follow up appointment at a convenient time that works best for you. Patients are generally asked to be seen within 1-2 days and are provided with a complete report of findings. Further treatment options and recommendations will be discussed during this time.
